Submit this form to make a request for an installment plan under section 5(a) of the Installment Policy (Policy number 2022/G-8).
Once we receive your request, we will request you to pay a commitment payment determined in accordance with the Installment Policy. An installment plan will be granted if you pay the commitment payment within the period specified in our notice, subject to other relevant conditions specified in the Installment Policy.
